4-H Volunteer Monthly Report 2023
Please complete this form at the end of the month to report on that month's 4-H data. This form should be turned in by the 10th of the next month. (If reporting for January, form must be completed for January's data by Feb. 10).

ALL enrolled Rowan County 4-H volunteers should complete this form, no matter how you contribute as a 4-H volunteer. Cooperative Extension requires that I report how many contact my 4-H volunteers had and how many hours they have spent volunteering for 4-H.

You know better than I do how much time you spend preparing, leading, and learning as a 4-H volunteer! I appreciate you taking time to fill out this form so that my reports can be accurate!

Use this formula: # of youth who participated in service project X how many hours service project lasted = total # of service hrs
