This form is used by faculty to report alleged academic integrity violations. If a faculty member suspects academic misconduct has occurred, they should follow the steps listed on the Office of Student Conduct website to resolve the matter.

Procedures used to resolve academic misconduct cases are available in the Student Discipline Procedures, which accompanies the Code of Student Conduct. Appendix B addresses these procedures and can be found at: Student Discipline Procedures.
The Office of Student Conduct encourages faculty to use this online form to upload all relevant materials, including:
- All materials related to the alleged violation;
- A copy of the course syllabus;
- Any relevant communications between the faculty and the student
