Chowan County 4-H Summer Fun 2021
4-H Summer Fun is a variety of engaging and educational workshops offered by the Chowan County Cooperative Extension Office. Please complete one form for each youth you are registering. Workshops are on a first-come first-serve basis. If classes fill up, you will be added to a wait-list. Prior to each workshop, parents/guardians will receive an email with program details, as there are some Covid-19 protocols we are required to follow, per North Carolina State University.

In addition to this form, an active 4-HOnline account is REQUIRED to participate in Chowan County 4-H programs. If you already have a 4-HOnline account, you only need to re-enroll for 2021. If you do not have a 4-HOnline account, please create a family account and add each youth as a member of your family. Your registration is not complete until your account is active. 4-HOnline can be accessed by visiting: https://v2.4honline.com

Payment is due prior to attending each workshop. Checks or exact cash will be accepted. Please make checks payable to Chowan County 4-H. You may drop payment off at the Chowan County Extension Office (730 N. Granville Street, Edenton). Refunds will not be given unless a class is canceled by the Extension Office.
