This form should be used if you received an error or require an override for Fall 2021 Statistics distance education courses that you qualify for, need permission for, or otherwise need assistance with during registration. Otherwise, if you can register on your own please do so by visiting MyPack portal within your designated enrollment dates (https://studentservices.ncsu.edu/calendars/enrollment/).

NOTE 1: NCSU students are required to use their University email address while completing this form. If you are a NCSU student, please log off all other email accounts before attempting to use this form. Students from outside the University should use an email account that will be checked often for correspondence from our team.

NOTE 2:  All information is important. It may take a minimum of than 24-48 hours to get your request(s) into our registration system in certain cases. We may also need to verify prerequisites have been satisfied with a review of unofficial transcripts, if necessary.

NOTE 3: Advisory holds require you to (1) set up a meeting with your academic advisor to discuss your options for the upcoming term. And (2) For master's students this may include your Graduate Plan of Work being completed prior to registering. Complete this form after all account holds have been removed.
